RIP cinematographer Halyna Hutchins; and speedy recovery to director Joel Souza.
How did this happen? They're still investigating, but this article (What are prop guns and why are they dangerous?) sheds some light on the general parameters of using "prop guns" on a movie set, including mention of a few other notable fatalities from on-set mishaps.

Honestly this is the prop masters/armorers mistake. Human negligence at its best. Some reports say it was a live round, which isnt allowed on movie sets so I dont see that happening. More than likely a piece of frag metal got stuck in the barrel and the next shot got it propelled down the barrel. Same thing happened to Lee on the set of the Crow. Either way its a tragedy.
