In 19th century England, a noted brain surgeon rescues a former student from being hanged on a false conviction for murder, and spirits him away to an ancient, repurposed abbey far in the countryside. There, he connives his pupil into assisting him in mapping the functions of the various parts of the human brain, using living subjects who are under a terrible animation-suspending drug called "black sleep". Subsequently, the student, along with the daughter of one of the subjects, discover that most of these subjects have survived but are being kept in a dungeon-like cellar, in various stages of physical and mental derangement...

Durch eine Droge in totähnlichen Schlaf versetzt kann Ramsay dem Galgen entkommen und soll als Gegenleistung helfen, Thostis schwer kranke Frau zu heilen. Der so gerettete Ramsay muss jedoch schon bald feststellen, dass seine Situation nicht wirklich entspannt ist. Thosti experimentiert an Menschen und hält verunstaltete Versuchsobjekte gefangen, mit denen er skrupellos umgeht. Um seine Frau zu retten, hat der Mediziner alle moralischen Bedenken über Bord geworfen und lässt sich von seinem Weg nicht abbringen, der nur ins Verderben führen kann.

Nell'Inghilterra di fine '800, il dottor Gordon Ramsey è stato condannato a morte nel carcere di Newgate per un omicidio che non ha commesso. Il dottor Cadman, suo antico maestro, riesce tuttavia a salvarlo somministrandogli una droga - il "sonno nero" - che provoca una morte apparente. Sfuggito alla condanna, Ramsey accompagna Cadman in un antico maniero e accetta di collaborare ad un folle esperimento di trapianto di cellule nel cervello della moglie del medico, irrimediabilmente malata. Nei sotterranei del castello sono situati il laboratorio e le prigioni che ospitano un gruppo di povere creature rese idiote e deformi dalle precedenti fallimentari operazioni tentate da Cadman. Ramsey si accorge ben presto che le scelte dello scienziato non rispondondo propriamente ai dettami dell'etica professionale, ma, vittima degli effetti del "sonno nero", rimane involontariamente coinvolto nella tragica vicenda.

Londres, 1872. En su celda de la Prisión de Newgate, el doctor Ramsay recibe la visita de sir Joel Cadman, su antiguo mentor, que le ofrece la libertad a cambio de que sea su ayudante personal.
