In the big stand off, Alabama wrote on a napkin, prompting Clarence to go to the bathroom, where he missed most of the shoot out.

What did she write, and why?

"You're so cool"

Why? Because Clarence was cool.

This is from an early draft (August 1992) of the script (page 116), the why is pretty obvious?

"Lee continues looking at the merchandise. Alabama writes something on her napkin with a pencil and slides the napkin over to Clarence. It says: "You're so cool" with a tiny heart drawn on the bottom of it. Clarence takes a pencil and draws an arrow through the heart. She takes the napkin and puts it in her pocket."

Not obvious to me how "You're so cool" means "go to the bathroom and avoid the bloodbath."

Why are you equating the note with going to the bathroom? It doesn't have to be connected. But you are welcome for the info you asked for.

The way the scene was filmed, they appear connected to me. I'm betting it's an editing issue, frames got chopped somewhere in the movie that might better have set up that particular sequence. Sure I could be wrong, but does that napkin writing thing make any sense to be shown at that time for no reason at all?

I never read the script or researched it online, so it must have been in the versions I have watched multiple times.

Here's one explanation from a critic:

“You’re so cool. You’re so cool. You’re so cool.” This lively song plays throughout the film bringing a vibrant, cheerful mood inciting joyful feelings in the viewers. They feel instant happiness and are optimistic that something positive will follow. These three words also carry a deeper meaning between Clarence and Alabama. Alabama writes “You’re so cool” with a heart on a napkin for Clarence during the cocaine exchange (and he finishes the heart by drawing an arrow through it) as a reminder of how deeply she loves him. This repeating and very significant quote carries a stronger purpose than what it would mean in any other context. When Alabama writes “you’re so cool” on the napkin, it insinuates her love for him without actually saying, “I love you.” It is an important and unique aspect in this film because it stands out in the end. Because of the repetition of the song “You’re so cool. You’re so cool. You’re so cool,” it becomes memorable to the viewers not only as a vibrant and jovial song but also as an unforgettable quote from the film. As previously stated, “True Romance” is not your typical love story, so by using “You’re so cool” to represent “I love you,” it continues to emphasize that this film is different and more memorable than your ordinary romance movie as it keeps things romantic, entertaining, and distinct."

If that's all it is, well, then, so be it. The timing still seems odd to me but, hey, I guess that's my hang-up.
